Incident Reporting: Incident Response

The Direct Support Professional secures the area and notifies the DMT manager or the on-call manager. Please reference Appendix C regarding the accident reporting response.

The Direct Support Professional (DSP) submits the Incident Report Participant/Witness form within 4 hours to the Division Management Team, who notifies the appropriate people regarding the incident. If at night or on the weekend, staff must have the form submitted by noon of the following day. Failure to do so may result in disciplinary action.

Incident reports should include Who (who did it happen to), What (what happened), When (when did it happen), Where (where did it happen), and the outcome (how was the incident resolved).
