Mandated Reporting

All Employees of ASN are considered mandated reporters and are required to report any suspicions of abuse, maltreatment, neglect, or bullying. The Child Protective Service hotline number is 800-482-5964. The Adult Protective Service hotline number is 800-482-8049.

Incident Reporting: Reportable Events

The following list are examples of occurrences that are deemed IMMEDIATELY reportable. If you are ever in doubt about whether a situation is reportable, err on the side of caution and report the situation IMMEDIATELY to your supervisor. This is an example of an incident report: > On 11.09.2021 at 10:45 pm member came out of his apartment and slammed his door. Staff 1 came outside to redirect the member. Member was upset at his video game. > > Member called staff 2 and walked back inside his apartment. > > Member became verbally aggressive with staff 1 and staff 2 was able to deescalate member. > > No further incident.

Incident Reporting: Incident Response

The Direct Support Professional secures the area and notifies the DMT manager or the on-call manager. Please reference Appendix C regarding the accident reporting response. The Incident Debrief Form will be completed by the DM for the employee who reports an incident that involves: - CPS - APS - Medication errors - Law enforcement - Health and safety - Hospitalization The form will be used as a coaching opportunity to address the incident details, outcomes, opportunities to prevent and better manage future incidents.
