Part II: Signing and Submitting an Activity Record in Setworks

1. Adding Overall Comments

  • Enter a description of your shift and the activities you completed on the shift.

  • Select Save Comment

2. Individual Comments

  • Click on the arrow next to the name of the person you support to open up the individual documentation
  • If you are working a shared shift and supporting more than one person on the shift, each individual will be listed in your ACTIVITY RECORD, and you will need to click on the arrow next to each person's name and complete the individual documentation for everyone supported on the shift

 

3. Updating Additional Fields

  • You must update Goals and Objectives to submit the Activity Record for the shift

  • You will update the Clinical Events, Additional Supports, and Goals and Objectives in the following steps.

4. Additional Supports

  • THIS IS THE MOST IMPORTANT SECTION OF THE ACTIVITY RECORD. 

  • Additional supports documents the importance of your role as a Direct Support Professional and provides justification for why the person needs direct support

  • If you are not documenting the supports your provide in the ADDITIONAL SUPPORTS section, you are not justifying the support services you provide

  • From the drop-down menu of supports, select EVERY support you provided to the individual on the shift

  • Select all that apply

  • If none apply, skip, and continue to the next step

 

5. Clinical Events

  • Select Add/View Clinical Event to add a new clinical event
  • Update as needed by selecting Add Record
  • Update the following fields when adding a new Clinical Event record 

    1. Date/Time

    2. Active

    3. Asleep?

    4. Safety Check Completed?

    5. Concerns/Observations Notes

  • Select Update to save your new entry 

  • Continue to the next step
     

6. Goals and Objectives

  • Select Goals/Objectives

  • You will need to complete the GOALS/OBJECTIVES section for each individual you support on the shift

  • If you are working a shared shift and supporting more than one person on the shift, each individual will be listed in your ACTIVITY RECORD, and you will need to click on the arrow next to each person's name and complete the individual documentation for everyone supported on the shift

  • Some support teams will need to complete documentation in the CLINICAL EVENTS and PROTOCOLS/CUSTOM PLAN QUESTIONS sections, but not every team

  • If you are required to complete CLINICAL EVENTS or PROTOCOLS/CUSTOM PLAN QUESTIONS DOCUMENTATION, you will receive additional training on how to complete those sections

  • Describe how the person you support performed/felt/engaged during the shift

  • You can include specific activities, conversations, challenges, and/or achievements the person had during the shift

  • Select Save Comment 

  • Each goal will provide you with: 

    • Goal - The category or area of need 

    • Goal Detail - The specific action or thing the individual receiving supports wants to accomplish 

    • Method/Techniques/Strategies  - Instructions for the DSP to support the individual in achieving their goal 

    • Objective - The small action steps you will focus on each shift to support the individual in achieving their goal

    • A goal can have multiple Objectives

You must complete documentation for every goal and objective in a person's plan before you can sign and submit an Activity Record.

7. Assessment Keys

  • Select one of the following to indicate the progress the individual made on the Objective compared to previous attempts:
    • PM - Progress Made means today the person performed better than they have before
    • NC - No Change means today the person performed the same as they have in the past
    • R - Regression or Refusal means today the individual's performance declined compared to past performance or the individual refused to complete the objective
    • NA - Not Applicable on Shift means this objective was not covered during your shift 
    • Add any comments or additional information necessary to describe the OBJECTIVE and the person's performance on this specific OBJECTIVE in the comment box

    • Select Save Comment
       

8. Intervention Keys

  • Select all that apply to describe the types of supports you provided to the individual to assist them in completing the OBJECTIVE

  • Select all of the following that apply:

    1. I - Independent means the person completed to objective without any prompting, assistance, or support from you 

    2. V - Verbal Prompts means you gave verbal reminders, instructions, or information to assist the person in completing the activity 

    3. G - Gesture Prompts means you provided gestures to instruct and assist the person to complete the task 

    4. M - Modeling means you modeled or demonstrated to activity for the person 

    5. P - Physical Assistance means you physically assisted the person to complete the task 

    6. T - Total Assist means you completed the task for the individual without any involvement from them 

    7. NA - Not Applicable on Shift means the activity did not occur on this shift 

  • Add any comments or additional information necessary to describe the OBJECTIVE and the person's performance on this specific OBJECTIVE in the comment box

  • Select Save Comment

9. Reviewing the Activity Record for Completeness and Clarity

  • When Complete, the Goals and Plan Questions will appear in Green

  • When incomplete, the Goal and or Plan Question will appear in Red and will show as Incomplete

  • Select Role

    1. Select a role to assign the activity record to

    2. Assign as Staff

 

10. Save and Sign 

  • Select Save and Sign
  • Electronically Sign the Activity Record by entering your Setworks PIN

  • Submit the Activity Record
