Christmas Club Account Setup Instruction
Please follow these steps carefully to open your Centennial Christmas Savings Account:
1. Complete the Enrollment Form
- Ensure the Centennial Christmas Savings Account Form is filled out in its entirety.

2. Gather Required Identification (Two Forms)
- Primary ID (ID #1): A valid (non-expired) Driver's License or State ID.
- Address Verification: If the address on your primary ID is not current, you must also provide a recent Utility Bill for proof of address.
- Secondary ID (ID #2): Provide one of the following: Social Security Card, Passport, Employee ID Badge, Military ID, Birth Certificate, or Insurance Card.
3. Submit Documentation
- Scan the completed form and both IDs (plus proof of address, if required).
- Email all documents to: asnhrdepartment@supports.org
Next Steps & Important Information:
HR will promptly submit your enrollment form and requirements to the bank. Please note the following:
- HR or the bank may reach out to you directly for clarification or additional details. The timely setup of your account is dependent on your availability to respond.
- Payroll deductions for your account will only begin after the bank has successfully opened your account and provided us with an official account number.
- Please be advised that contributions to your Christmas Club Account may take up to 90 days to be processed and reflected in your account.
