All employees who will transport individuals we support must keep current auto insurance for their vehicle(s) on file, as well as their driver's license.
If a DSP will not be transporting an individual, the insurance and driver's license can be waived, provided we have a statement on file from the individual's guardian or responsible party. Please communicate with Human Resources if you think you need this.
You can submit scans, photos, or electronic documents, via ticket or email.
If your name is not on the auto insurance that you provide, you will also need to submit this form, completed by the policyholder. Click here to download a blank version of the letter for them to fill out.
