Criminal Background Check

All ASN employees must complete the criminal background check every 5 years. See below for the form; use the arrow at the top right to open and download it. The instructions on the form say that it must be notarized and you need to pay a fee online. Human Resources will notarize it and the fee is payroll deducted; all you have to do is fill it out and turn it in to ASN.
